Think about a typical week over the last month. For each day, fill in the number of standard drinks of each type of alcohol you typically consumed on that day, and number of hours you spent drinking. (If you did not drink at all on that day, enter 0.)
One “standard drink” is about:
- 12 ounces of beer (5% alcohol) or
- 5 ounces of table wine (12% alcohol) or
- 1.5 ounces of 80 proof liquor (a “shot” of 40% alcohol)
